the question specifies that no matter what the pilot vessel is on pilotage duty. the pilot could be still on duty while in A B or C, therefore D must be correct because whenever a pilot vessel is on duty it is required to show identity lights

Alongside a vessel implies that it is engaged in pilotage duty. The other answers may not be engaged in pilotage duty and in that event would not have identity lights on. It's a bad question in my opinion.

I think this question is wrong. "Shall" implies has to have lights on.
If underway and not engaged in pilot age duty it would not show pilotage lights, same as at anchor.
